What Is Original Medicare?

Original Medicare is the federal program made up of Part A (hospital insurance) and Part B (medical insurance). It covers a broad range of services and lets you see any doctor or specialist in the country who accepts Medicare, with no referrals required. That flexibility is a real benefit for people who travel frequently or who have established relationships with specialists in different cities.

The trade-off is that Original Medicare does not cap your out-of-pocket costs, which can feel risky if a major health event comes up. Many people pair it with a Medicare Supplement plan (also called Medigap) to cover those gaps, along with a separate Part D plan for prescription drug coverage.

How Medicare Advantage Works Differently

Medicare Advantage, or Part C, is offered through private insurance carriers approved by Medicare. These plans bundle your Part A and Part B coverage together, and most include prescription drug coverage as well. Many also throw in extras like dental, vision, and hearing benefits that Original Medicare does not cover.

  • Cost structure: Medicare Advantage plans often have lower monthly premiums but may have higher out-of-pocket costs at the point of care. Original Medicare paired with a Medigap plan tends to have higher combined premiums but more predictable costs when you use services.
  • Extra benefits: If dental, vision, or fitness perks matter to you, Medicare Advantage plans frequently include those. Original Medicare does not.
  • Prescription drugs: Most Medicare Advantage plans include Part D. With Original Medicare, you add a standalone Part D plan separately.
